[Labor Accident Case] Organic solvent poisoning during the drying process of pharmaceutical intermediates.

We will introduce a case where organic solvent poisoning occurred during the drying process of pharmaceutical intermediates.

We will introduce a case of drying work involving a pharmaceutical intermediate containing a large amount of acetone as a solvent at a pharmaceutical manufacturing plant. In the production of drugs, after the filtration process, workers moved the intermediate to the drying room, transferred it to trays, and placed the trays on the shelves of a low-temperature dryer, during which they were exposed to high concentrations of acetone. After finishing work in the drying room, two workers reported feeling unwell and were taken to the hospital, where they were diagnosed with organic solvent poisoning and received inpatient treatment. 【Suggestions】 - Workers should wear personal PID monitors while working. - This will allow for monitoring exposure levels and assist in chemical risk assessments. *For more details, please refer to the PDF document or feel free to contact us.*

[Labor Accident Case] Carbon monoxide poisoning that occurred in a pump well at a water supply intake facility.

Introduction of a case of carbon monoxide poisoning that occurred in a pump well at a water supply intake facility.

Three workers were engaged in the replacement of the backflow prevention valve for the water supply pipe installed in the pump well. While using a submersible pump powered by a gasoline engine to draw water, exhaust gases accumulated in the pump well, causing the three workers to report feeling unwell and lose consciousness. Two victims who remained in the pump well were rescued by staff from the water purification plant. One individual managed to escape on their own and all three were transported to the hospital, where they were hospitalized for 11 days. 【Suggestions】 - Use gas detectors to confirm the effectiveness of ventilation. - Work while ensuring safety with a portable gas monitor. *For more details, please refer to the PDF document or feel free to contact us.*

[Labor Accident Case] Chlorine gas was generated during cleaning work in the vegetable washing room.

We will introduce a case where chlorine gas was generated during cleaning operations in the vegetable washing room.

A sodium hypochlorite was taken out from the container in the sterilizing water generation device, and after removing mold from the walls and equipment in the cleaning room, the remaining sodium hypochlorite was accidentally placed into the neighboring hydrochloric acid container, resulting in the generation of chlorine gas. There were no individuals who felt unwell during the evacuation and handling due to the chlorine gas generation, but after returning home, a worker felt unwell and was transported to the emergency room. 【Suggestion】 ■ In addition to risk reduction measures, it is recommended to work while ensuring safety with a gas detection alarm. *For more details, please refer to the PDF document or feel free to contact us.

[Labor Accident Case] Hydrogen Cyanide Poisoning Occurred During Research Work

I will introduce a case where cyanide poisoning occurred during research activities.

During the post-processing stage of producing the adsorbent, after removing bromine cyanide from the water, sodium hypochlorite was added for treatment, resulting in the generation of hydrogen cyanide due to a chemical reaction. Since no exposure prevention measures were taken, the worker was exposed to hydrogen cyanide. About 30 minutes after finishing the work, the worker felt nauseous and went to the hospital, where they were diagnosed with cyanide poisoning and admitted. 【Suggestion】 ■ In addition to risk reduction measures, it is recommended to work while ensuring safety with a gas detection alarm. *For more details, please refer to the PDF document or feel free to contact us.

[Labor Accident Case] During a blast furnace test at a steel mill, CO gas leaked inside the furnace, causing multiple cases of poisoning.

We will introduce a case where multiple people were poisoned due to a leak of CO gas inside the blast furnace during the testing of measurement devices at the steelworks.

In the electrical room, the main power was turned on, the main circuit switch at the site was turned off, and when the "forward" button on the measuring device was pressed, the device retreated due to the gas pressure in the furnace. After pressing the "stop button," the measuring device temporarily stopped, but then when the purge button for the antenna into the furnace was pressed, the measuring device retreated further. Gas from the furnace leaked through the tire seal section of the ball valve that was stopping the gas leak, causing many people nearby and those watching the test from a distance to be exposed to the furnace gas and suffer from carbon monoxide poisoning. [Proposal] ■ Workers should wear portable gas monitors while working. ■ This will enable early detection of gas leaks from the furnace and alert workers and those nearby to the danger. *For more details, please refer to the PDF document or feel free to contact us.

[Labor Accident Case] Hospitalized due to pouring waste liquid into a waste liquid tank filled with water.

A case where placing waste liquid into a waste liquid tank filled with water resulted in chlorine gas poisoning and chemical conjunctivitis.

During the research process of the spice, the victim placed waste liquid containing phosphoric chloride, which was used as a reagent, into a waste tank filled with water for fire prevention. This caused a reaction with the water in the tank, resulting in the release of chlorine gas, leading to exposure. The victim was transported to the hospital by ambulance and was diagnosed with "chlorine gas poisoning and symptoms of chemical conjunctivitis," resulting in hospitalization. [Recommendation] ■ In addition to risk reduction measures, it is recommended to work while ensuring safety with a gas detection alarm. *For more details, please refer to the PDF document or feel free to contact us.

[Labor Accident Case] Incorrect disposal of used chemical solution resulted in the release of hydrochloric acid.

An incident where hydrochloric acid was generated due to improper disposal of waste liquid from chemicals used in a semiconductor manufacturing plant, resulting in harm to workers.

In the membrane pre-cleaning process of the manufacturing process, a mixed solution M of sulfuric acid, hydrogen peroxide, and pure water was used, while a mixed solution N of phosphoric acid and hydrochloric acid was used for the marking process. Although solutions M and N were processed separately, hydrochloric acid gas was generated when solution N was accidentally poured into the poly tank designated for collecting solution M. The hydrochloric acid gas, which erupted as white smoke from the poly tank, spread into the clean room, causing three workers inside to complain of eye pain and seek treatment. [Recommendation] ■ It is recommended to install equipment that can quickly detect the leakage of toxic gases and issue alarms. *For more details, please refer to the PDF document or feel free to contact us.
